how to build a form using more than one table?

Hi Arabella, Thank you for the post, In answer to your question; to undertake this you will need to establish relationships between your tables, ie 1 - 1 , 1 to many, many to many, and then create a query which has the fields in it from the tables that you wish to use.
From this query you can then use the Form Wizard or create the Form manually in Design view, placing the fields where you wish them to be on the Form.
